Adult Day Services is a licensed program of supervision, medical monitoring, nutrition, socialization, transportation, and just plain fun!

We offer an alternative to nursing home care when less than 24 hour care is needed.

We offer respite care to families so they can continue employment and other activities not possible when providing full-time care.

Our team is trained to care for clients with dementia and other disabilities in a comfortable, homelike environment.

What services are offered?

Our program is available Monday—Friday, 8:30 a.m. to 3:30 p.m. A van with a wheelchair lift provides transportation to and from your home in Crookston. Participants receive a light breakfast, a well balanced noon meal, and a snack before returning home.

Referral and Enrollment

Each participant must meet with the program director to determine if the program is appropriate and to complete admission requirements. Days of attendance are scheduled according to availability and the individual’s needs. The daily fee can be paid privately, or if the individual qualifies, financial assistance is available through Polk County Social Services.
